I have read the Policies and Procedures of the Excel Swim School and agree to follow them. I understand that I must also mail payment and a signed release form to secure my spot in a class. Excel Aquatics requires that you submit this online registration form with your class preference and also mail a paper copy of this form in the same envelope with your check and signed release form to the address below.
